Never move on assumption

The most avoidable harm in critical care transfer is a patient loaded into an ambulance before a bed is confirmed. In Lagos traffic that can mean two hours each way. Get a named acceptance first.

Stabilise before transfer

Secure the airway if it is at risk
Ensure oxygenation is adequate for the journey
Address circulatory instability with fluid or vasopressors
Treat a critically high potassium before departure
Confirm the vehicle carries oxygen, suction, monitoring and pumps
Send a competent escort with a ventilated or unstable patient

Send with the patient

Referral letter with diagnosis and treatment given
All results with times taken
Imaging on disc or as photographs
Electrocardiogram where relevant
Medication and infusion list with rates
Hepatitis and HIV screening status
Identification, HMO details and a relative

Availability changes hourly

We do not publish a live bed count, because a number that is right at nine in the morning is misleading by noon. Call and you will be told immediately whether we can take your patient. If we cannot, we will say so rather than leave you waiting on a decision.
