Where anything here appears to conflict with what the clinical team tells you about your relative, the clinical team is correct, because they have the information that this website does not.

The referral guidance describes our processes and general principles. It does not replace your own clinical judgement about whether and when a patient is fit to transfer, and it is not a protocol for management in your own institution.
